[New feature] AND Search

In PowerQA, AND search is now available.

This is an experimental implementation.
Search word is limited to 5 words in default. If you hope to change this restriction, you have to change define in qa-include/qa-db-maxima.php.
@define('QA_DB_MAX_AND_SEARCH_LENGTH', 5);
Caution: If this number is increased, search performance will be degraded.
Search results are sorted in the order of the new question.
